Office Policies for Living Healthy Body and Mind Mental Health Clinic

At Living Healthy Body and Mind, we are committed to providing high-quality, compassionate care in a safe and respectful environment. The following policies are designed to ensure a smooth and professional experience for all our patients.

Privacy and HIPAA Compliance

  • Privacy: We take your privacy seriously. All personal and medical information is kept confidential and secure.
  • HIPAA Compliance: We strictly adhere to the Health Insurance Portability and Accountability Act (HIPAA) to protect your health information.


Medication Refill Policy

  • Refill Requests: Please allow 48 hours for medication refills. Refills can be conveniently requested on our website.
  • Controlled Substances: All controlled substance prescriptions are limited to a maximum of 30 days per fill at the designated pharmacy. No replacements for lost, stolen, or expired medications will be provided.


Appointment Policy

  • Late Policy: If you are more than 10 minutes late for a telehealth appointment, the visit will be canceled and may be subject to a no-show fee.
  • Provider Delays: If your provider has not logged on within 10 minutes of your scheduled appointment time, please contact our office immediately.
  • No-Show Policy: No-shows result in a fee. After two no-shows, you may be discharged from the practice.
  • Payment Due: Payment is due prior to your visit. We accept credit cards, HSA cards, and offer Advance Care financing options.


Insurance and Payment Methods

  • Insurance: We accept most insurance plans. Please verify your coverage with our office before your appointment.

Outstanding Balances

  • Policy: Outstanding balances must be settled before scheduling future appointments.


Follow-Up and Continuity of Care

  • Continuity: Regular follow-up appointments are essential to ensure the best outcomes for your mental health care. Please adhere to your scheduled visits.


Crisis and Emergency Situations

  • Crisis Support: In case of a mental health crisis, contact the Suicide & Crisis Lifeline at 988 for immediate support.

Controlled Substance Policy

  • Prescription Monitoring: Patients using controlled substances on a continuing basis must attend regularly scheduled office or virtual visits.
  • Controlled Substance Agreement: Patients prescribed Class II or ADHD/ADD medications must sign a Controlled Substance Agreement.
  • Prescription Policy: Receiving controlled substances from another provider while under our care is grounds for immediate termination of the provider-patient relationship. We will provide care for 30 days following the outside prescription, during which time you must find a new provider.
